Consideration of Intradural Nerve Root Enhancement on Contrast Enhanced MRI

Abstract
Intradural nerve roots are not enhanced in contrast-enhanced MR images because of the presence of blood-brain barrier. However, the enhancement of nerve roots has often been encountered. It can be the enhancement of the normal anatomy such great or accessory radicular vein, or can be the MR findings of the diseases involving nerve roots. The disorders showing the enhancement of nerve roots are herniated disc, spinal canal stenosis, postoperation due to lumbar disc problem, infection such as tuberculosis, malignancy such as lymphoma, leukemia or other metastasis, and miscellanies such as Guillain-Barre¢¥ syndrome. The nerve root enhancement, caused by compressing the nerve roots such as herniated disc or stenosis is related to the pain. Spinal tuberculosis and leptomeningeal involvement of malignancy tend to show the irregular and uneven enhancement of nerve root and viral radiculitis and idiopathic autoimmune disease such as Guillain-Barre¢¥ syndrome tend to show the smooth and even enhancement of nerve roots. However, the differential diagnosis of nerve root enhancement sometimes can be very difficult because of the similarity of the clinical and imaging features.
